2) Why is this survey being conducted? What is the purpose of the study?
The Boston Public Health Commission’s mission is to promote and protect the health of everyone who lives in the city of Boston. We are collecting important information about the health and well-being of children to help better serve the needs of all children in the city. These data will be used to improve and direct child health programs and services, to communicate our findings on the status of children’s health to caregivers, policymakers and community leaders, and to support funding requests for improving the health of Boston’s children.
3) What is this survey about?
This survey asks some general questions about your child’s health, such as if they have been told they have asthma or allergies. It also asks about behaviors that affect their health, such as nutrition, exercise habits, eating habits, frequency of doctor’s visits, etc. If there are questions you wish not to answer, you can refuse an individual question and still participate in the study as a whole.

The survey should be completed by the parent or guardian who knows the most about the health of the child or children in the household.
